Short version: what you type into the checklist form, and nothing else identifying.
If you submit the checklist form we receive the state you are licensed in, the date you entered, and your email address. It is used to send you the checklist. We do not sell it.
The decision path on each state page runs entirely in your browser. Your answers are stored in your own browser so the page can restore your result, and are not transmitted unless you submit the form.
