Provide License Verification
▾
Oklahoma requires official verification of your active out-of-state license. This must be sent directly to the board from your current state board or its official online verification system before your file can be considered complete.

Provide CDR Registration Verification
▾
Proof of active registration with the Commission on Dietetic Registration sent directly to the Board.

Request Official College Transcripts
▾
Sent directly from the university showing your conferred degree.

Provide Extended Background Check
▾
Processed through the Oklahoma State Bureau of Investigation (OSBI).

Provide Evidence of Legal Status
▾
Form documenting your citizenship or qualified alien status.

Complete Fingerprinting and Background Screening
▾
Oklahoma requires a criminal background check for all applicants. Out-of-state applicants can use IdentoGO Out-of-State Live Scan (adds ~$39.95 fee) OR FD-258 hard card via mail (tip: moisturize hands for days prior to reduce smudge rejects). Completing prints outside Oklahoma can add 2-4 weeks to your processing time.

Await Board Processing
▾
Plan for a processing timeline of about 4 to 6 Weeks once your file is complete. If you need to start working immediately, most states do not offer temporary practice authorization during the endorsement process. Contact the state board directly for current policies on practice authorization while your application is pending.

Create an Account
▾
Register for a new account on the OKLAHOMA MEDICAL BOARD REGULATORY SERVICES PORTAL at https://oklahoma.professions.portal.infor.com/. Provide your personal details, including SSN and date of birth, create login credentials, and verify your email address.

Upload Supporting Documentation
▾
Upload all necessary documents as specified in the Dietitian Initial Application Instructions, including official transcripts, verification of registration with the Commission on Dietetic Registration (CDR), and a recent passport-style photograph, directly through the OKLAHOMA MEDICAL BOARD REGULATORY SERVICES PORTAL.

Board Review and Approval

Your complete application, including all submitted documents and background check results, will be reviewed by the Board staff. Once all requirements are met and approved, your Oklahoma Dietitian license will be issued.
