How to Apply

1. Create a Nurse Aide Registry Account ▾
   Navigate to the Nurse Aide Registry Management System at https://apps.health.ok.gov/NurseAide/login.aspx and click "Create New Account" to establish your applicant profile if you are a new user.
2. Complete the Online Application ▾
   Log into your Nurse Aide Registry Management System account, select the appropriate application type (e.g., Initial Certification, Reciprocity), and accurately fill out all required personal, educational, and work history information.
3. Upload Required Documentation ▾
   Submit all necessary supporting documents, such as proof of training, competency exam results (if applicable), and any other required forms, directly through the Nurse Aide Registry Management System.
4. Pay Application Fees (if applicable) ▾
   If your application type requires a fee (e.g., reciprocity), process the payment online using a credit or debit card within the Nurse Aide Registry Management System. Note that initial certification for new graduates from approved programs typically has no fee.
5. Schedule and Pass Competency Exam (if required) ▾
   If you are pursuing initial certification without prior licensure, ensure you schedule and successfully pass both the written and skills portions of the Oklahoma state nurse aide competency examination.
6. Monitor Application Status ▾
   Regularly check the status of your application through your account in the Nurse Aide Registry Management System for updates or any requests for additional information from the Nurse Aide Registry.
