How to Apply

Complete a Board-Approved Nitrous Oxide Monitoring Course ▾Successfully complete a Board-approved course specific to the monitoring of nitrous oxide administration. Ensure the course meets the requirements outlined in Title 172 NAC 71 of the Nebraska Rules and Regulations for the Board of Dentistry.

Download the Certification Application Form ▾Obtain the official 'Dental Assistant - Nitrous Oxide Monitoring Certification Application' (Form 16-21) from the Nebraska Department of Health and Human Services (DHHS) Dentistry Forms page.

Complete Application and Gather Documentation ▾Fill out the application form completely and accurately. Attach all required documentation, including official proof of completion from your Board-approved nitrous oxide monitoring course.

Prepare Certification Fee ▾Include the non-refundable initial certification fee of $25. This fee should be submitted with your completed application.

Mail Application and Fee ▾Mail the completed application form, all supporting documents, and the fee to the Nebraska Department of Health and Human Services, Licensure Unit, at PO Box 94986, Lincoln, NE 68509-4986.

Receive Certification ▾Allow time for processing by the Licensure Unit. Once approved, the Board will issue your Nitrous Oxide Monitoring Certification.
