Transfer License

Steps

1. Submit Your Application ▾
   Apply through the Kentucky Board of Licensure and Certification for Dietitians and Nutritionists. The application fee is $50.
2. Provide License Verification ▾
   Kentucky requires official verification of your active out-of-state license. This must be sent directly to the board from your current state board or its official online verification system before your file can be considered complete.
3. Confirm Your CDR Registration Card ▾
   Proof of active registration with the Commission on Dietetic Registration.
4. Request Official College Transcripts ▾
   Must show your conferred master's degree. Can be sent securely via Clearinghouse to the board.
5. Provide Supervised Practice Verification ▾
   Documentation showing completion of at least 1,000 hours.
6. Complete Fingerprinting and Background Screening ▾
   Kentucky requires a criminal background check for all applicants. Out-of-state applicants can use IdentoGO Out-of-State Live Scan (adds ~$39.95 fee) OR FD-258 hard card via mail (tip: moisturize hands for days prior to reduce smudge rejects). Completing prints outside Kentucky can add 2-4 weeks to your processing time.
7. Await Board Processing ▾
   Plan for a processing timeline of about 10 Business Days once your file is complete.
8. Create Kentucky Online Gateway (KOG) Account ▾
   Register for a KOG account to gain access to the Board's eServices Portal, which is required for all online applications.

Keeping Your License Active

Kentucky Dietitian licenses renew Annual (by Oct 31st) + 15 CE hours. Contact the Kentucky Board of Licensure and Certification for Dietitians and Nutritionists for continuing education requirements and renewal deadlines.
