How to Apply

  1. Create MQA Online Services Account
    Register for a new user account on the Florida Health Source portal to begin the application process.
  2. Submit Application and Fees
    Complete the online application for Licensure by Examination or Endorsement. Pay the non-refundable application fee ($80-$85) and initial license fee ($80).
  3. Complete Background Screening
    Undergo a Level 2 background check via an approved Livescan provider using ORI number EDOH4570Z. This is required for all new applicants as of July 1, 2025.
