Create MQA Online Services Account ▾Register for a new user account on the Florida Health Source portal to begin the application process.

Submit Application and Fees ▾Complete the online application for Licensure by Examination or Endorsement. Pay the non-refundable application fee ($80-$85) and initial license fee ($80).

Complete Background Screening ▾Undergo a Level 2 background check via an approved Livescan provider using ORI number EDOH4570Z. This is required for all new applicants as of July 1, 2025.
