How to Apply

    Submit the non-refundable application fee of $105 through the eLicense portal using a credit card or electronic check. Your application will not be processed until the fee is received.
  6. Await Department Review
    The Connecticut Department of Public Health will review your submitted application and all supporting documentation. You will be notified via the eLicense portal or email regarding the status of your application and any further steps required.
